Health Professionals Referral Form NRT
As a health professional you are in an ideal situation to offer smokers support to quit by simply referring them to the Local Stop smoking service. If you wish to refer a patient or client simply click on the referral for link and complete the form. The service will then contact the person within 24 hours (unless a referral is made on a Friday or Bank holiday weekend).
Barking and Dagenham Training Programme
The Barking and Dagenham training programme is based on two levels. Our focus is on training as many health professionals and voluntary organisations as possible to be Level 1 smoking cessation advisers as part of their role, and to extend the pool of Level 2 advisers within the local area. All our training sessions are FREE OF CHARGE and on the basis of first come first served policy.
Level 1
Primary health care teams are encouraged to keep up to date records of patients smoking and are advised to ask smoking status at least once every 12 months. A programme of level 1 training is offered to all those working in Barking and Dagenham. After completion of the training level 1 advisors should be able to undertake the following:
- Raise the issue of smoking and record smoking status appropriately
- Provide consistent and appropriate to all smokers
- Refer to local smoking cessation services
Individual feedback on the number of referrals, number setting a quit date and subsequently quitting is given to advisors.
Over the last 2 years we have incentivised level 1 advisors to refer patients to the Stop smoking Service. Financial incentives are based on activity and performances.

Level 2
Local in service training is provided for health professionals to become level 2 smoking cessation advisors and register as service providers. After completion of level 2 training advisors will be able to provide intensive 1 to 1 support and advice. They will receive remuneration plus additional bonus for activity and performances. Ongoing training and support is provided for level 2 advisors including annual update and regular newsletter. Individual feedback on the number setting a quit date and subsequently quitting is given to advisors.
